  2. When you are completing the application form be sure to read the terms and conditions, and attach any required documents as specified by the application instructions.
  3. If applicable, indicate how you will be funding your account (i.e. Bank Draft or Wire Transfer)
  4. Fax the signed form to DGM Bank & Trust Inc. at (246) 434 4850 (to speed up the application process). A DGM private banker will contact you upon receipt of your application.
  5. Send the original signed copy to DGM Bank & Trust after keeping a copy for your files.
